New Medical Administrator Coming Soon!

Coventry has been selected to provide both the administrative services as well as the network of providers to our plan members starting January 1, 2009.  MoDOT Employee Benefits staff is working hard with Coventry to make this transition as flawless as possible.  The website allows you to check providers only, but additional information and services will be added for the January 1, 2009 transition.

In order to receive your full benefit, Coventry needs current and accurate information about any other health benefits (including Medicare) that covers you or any family member.  Please complete the Coordination of Benefits Questionnaire and return to Coventry via fax at 800-803-5350 or call customer service with your information at 800-627-6406. 

Failure to complete and return by January 16th will result in denial of claims.

New Employee Enrollment

If you are a new Employee, you must enroll in SEBES within 31 days of employment with MoDOT/MSHP.  You can make changes to your coverage during the first 31 days of your hire date.  Coverage is effective the first of the month following receipt of the change.  After this time, you can only change plans if you have a change of address during the next Open Enrollment period.
